2020 - 2021 Oakwood Athletics

Ticketing allotments will be provided to each individual student-athlete. There are no plans at this time for ticket sales to the general public as our first priority is to ensure our student-athletes are able to compete in front of their family and close friends. There will be no passes, including staff, senior citizens and league passes.  Ticketing allotments will vary by venue and will be communicated to each team.

General Guidelines for Athletic Events Taking Place at Oakwood Schools

Registering for Final Forms

For Parent Registration:

1.  Go to https://oakwood-oh.finalforms.com/

2.  Click on NEW ACCOUNT under the Parent Icon.

3.  Type your name and email address, then click REGISTER.

4.  Check your email (you will receive a confirmation email in approximately two minutes).  Click CONFIRM YOUR ACCOUNT in the email text.

5.  Type and conform your password and click CONFIRM ACCOUNT.

6.  You will be asked if you want to grant another parent/guardian access to your registered student(s).  Either click SKIP THIS STEP or type the name and email address of the other parent/guardian and click ADD PARENT ACCOUNT.

7.  You account will be created and you can then REGISTER STUDENT for your first child.

For Student Registration

You will need:

  • Insurance company & policy number
  • Your email address and student email address
  • Doctor and dentist contact information
  • Hospital preference

1.  Go to https://oakwood-oh.finalforms.com/

2.  Click LOGIN under Parent Icon.

3.  Click REGISTER STUDENT.

4.  Type student's name, email address, date of birth, gender, high school graduation year and home address and click CREATE STUDENT.

5.  Assign your student to a sport by checking its checkbox.  After choosing all sports, click UPDATE SPORTS.

6.  Complete each form and enter your full name into the Parent Signature field at the bottom of the page. Click SUBMIT FORM and move to next form.

7.  When all forms are completed, you will see a "Forms Finished" message.  An email will be sent to the student's email, prompting him/her to sign forms requiring student signature.

8.  If you are registering additional students, click MY STUDENTS at the top of the page and jump back to Step 3.
